I approve of my child/ward’s attendance at the 2011 FIRE Football Camp and certify that he is in good health and able to participate in these athletic activities. I understand that participation in the FIRE Football Camp could result in an accident or injury. I certify that my child has been examined by a physician and found to be in good physical health and able to participate in all camp activities without restriction. Therefore I, as parent or guardian, assume all risks related to these athletic activities. Knowing the inherent risks, and in voluntary consideration permitting my child/ward to participate in the youth camp, I agree to release, indemnify, and defend FIRE Football Camp and their officers, employees, agents, volunteers, sponsors, and students from and against any claim which the participant, parents or legal guardian or any other person may have for any losses, damages or injuries arising out of or in connection with my participation. Furthermore, I further acknowledge that the camp director or staff associated with the FIRE Football Camp will not be liable for any damages resulting from any injury or illness my child or guardianship may sustain while attending camp. I understand that FIRE Football Camp does not provide camp medical insurance and that I am responsible for any and all medical expenses. I also understand that there are no refunds after July 10, 2011 for any reason, including weather cancellations.
